Merck and Moderna reported positive topline results on 19 August from INTerpath-001, a Phase 3 adjuvant trial of intismeran autogene plus KEYTRUDA in patients with completely resected stage IIB-IV melanoma [E1]. Intismeran autogene is the individualized mRNA neoantigen therapy also known as V940 or mRNA-4157, jointly developed by the two companies [E1]. The study met its primary endpoint of recurrence-free survival and a key secondary endpoint of distant metastasis-free survival compared with KEYTRUDA alone [E1]. KEYTRUDA monotherapy is the current standard in that adjuvant setting [E1].

Companies framed the result as a first: the first combination to beat KEYTRUDA alone on both RFS and DMFS in this adjuvant setting, and the first positive Phase 3 for an individualized neoantigen therapy and an mRNA cancer shot [E1][E2]. Those are claims about the trial’s place in the public record, not a regulator’s finding [E1]. The protocol is registered as NCT05933577 [E3].

Intismeran is built per patient. It encodes neoantigens sequenced from that person’s tumor and is given with pembrolizumab after surgery [E1][E3]. A Phase 2b study had already suggested a large cut in recurrence risk; INTerpath-001 is the confirmatory test in a larger adjuvant melanoma population [E1][E3]. Topline on 19 August does not include hazard ratios, absolute event counts, or a full safety table [E1]. Those details wait on a medical-meeting presentation and a journal paper [E1].

mRNA is no longer only a COVID franchise on this readout. Moderna’s COVID vaccine revenue has been the drag on the stock for two years; a positive Phase 3 in adjuvant melanoma is the first large oncology proof the platform can carry a different product [E1][E2]. Merck already sells KEYTRUDA into this setting, so the combination is an add-on to an incumbent [E1]. Whether payers will fund individualized manufacturing at scale is not answered by a topline RFS win [E1].

Tape reports treated the 6:45 a.m. Eastern release as a re-rating. Contemporaneous accounts said Moderna shares nearly doubled in the session, with Merck up as well [E2][E4]. Those prints are session colour, not a substitute for the endpoints [E1][E2]. No US approval, label, or launch date was attached to the announcement [E1].

What the companies have not shown is overall survival, a price, a manufacturing throughput number, or a filing timeline with FDA [E1]. A statistically significant RFS and DMFS result against the standard of care is the load-bearing fact of 19 August [E1]. Everything else, including how far the stock runs and whether other INTerpath tumour types follow, sits outside the topline [E1][E3].
