The Democratic Republic of the Congo had recorded 3,605 confirmed Bundibugyo Ebola cases and 1,587 deaths through 30 July, giving the outbreak a crude case-fatality rate of 44% [E1]. Confirmed transmission now spans 49 health zones in Ituri, North Kivu, South Kivu, Haut-Uélé and Tshopo, with 33 zones reporting cases during the preceding seven days [E1]. The total exceeds the 3,317 confirmed cases recorded during the country’s 2018–20 epidemic, making this DRC’s largest Ebola outbreak by confirmed cases, although that earlier Zaire-virus outbreak still caused more deaths [E1][E2].
That record arrived through a reporting system under severe strain. The confirmed tally rose by 1,481 cases after 17 July, but expanded testing and reconciliation of delayed records account for part of the increase [E1]. Most of the rise still reflects a real widening of the outbreak, according to the official assessment, so the corrected ledger cannot be read as a statistical mirage [E1].
Health workers have absorbed an unusually visible share of the failure: 151 have been infected and 44 have died [E1]. Those infections expose gaps in protective equipment, infection control, early recognition and safe patient handling across a response operating in active transmission zones [E1]. Community deaths, incomplete contact follow-up and new patients with no known epidemiological link indicate that surveillance is still finding portions of transmission late [E1].
Insecurity compounds each clinical weakness. Armed-group activity, displacement, attacks on health facilities and restricted access have interrupted testing, contact tracing, isolation and safe burials in affected eastern provinces [E1]. Ituri remains the centre, accounting for roughly 88% of confirmed cases, while mining, trade and population movement connect difficult-to-reach communities to provincial and international routes [E1]. WHO rates the risk very high inside DRC and high for neighbouring countries, while keeping the assessed global risk low [E1].
Medicine offers fewer shortcuts than it did during recent Zaire ebolavirus outbreaks. No licensed vaccine or specific treatment exists for Bundibugyo virus, leaving isolation, supportive care, contact tracing, safe burial and community engagement as the response’s principal tools [E1]. Candidate vaccines, antivirals and monoclonal treatments were still in early evaluation in July, with no mass campaign comparable to ring vaccination against Zaire virus [E1].
The central forecast assigns a 44% probability, with an uncertainty interval of ±11 percentage points, that DRC’s confirmed cumulative total reaches at least 5,000 by 30 September 2026. The call settles YES when a WHO Disease Outbreak News notice, Africa CDC briefing or DRC health-ministry situation report dated no later than that deadline publishes a cumulative DRC tally of 5,000 or more; otherwise it settles NO. The path requires about 1,400 additional confirmations in two months, a reachable accumulation while transmission remains active across dozens of zones and surveillance continues converting delayed infections into confirmed records [E1].
The strongest counter-case starts with the same reconciliation that enlarged the present record. A substantial share of older infections may already have entered the ledger, Uganda declared its domestic outbreak over on 28 July, and improved isolation or contact follow-up could bend new confirmations well below the July pace [E1]. A higher 61% alternative gives more weight to the small remaining numerical gap and the breadth of active transmission, but the wide uncertainty is warranted because the reporting curve mixes fresh infections with old cases arriving late. Congo has crossed its national case record without acquiring the vaccine that helped turn its previous epidemic.
