The Democratic Republic of Congo had recorded 2,536 Ebola cases and 1,033 deaths by Tuesday, according to the European Centre for Disease Prevention and Control. Forty-seven of 140 health zones had reported cases, and 738 patients were isolated or hospitalized. [E1]
Those figures already describe a large outbreak. They do not describe its full edge. World Health Organization director-general Tedros Adhanom Ghebreyesus said insecurity may have left the true burden more than twice as high and that health workers were following roughly four in five known contacts. A WHO update six days earlier counted 2,124 cases and 828 deaths across five provinces. It reported infections among 119 health workers and described restricted access, population movement and insecurity as obstacles to surveillance and care. The increase between public snapshots reflects both transmission and a reporting system catching up. [E3]
Contact tracing is the hinge. A team that cannot find one in five named contacts also cannot count people who were never named, fled an affected area or died outside a treatment centre. Case totals therefore combine an epidemic curve with the reach of the surveillance network. [E1][E3]
That does not license a precise multiplier. Tedros’s warning is an estimate of possible under-ascertainment, not a census, and weak access can distort trends in either direction. A sudden rise may mean faster spread, better detection or both. The clean discriminator is whether access improves while the share of contacts followed and samples tested rises. [E2][E3]
The national situation report makes the visibility gap concrete. Teams saw 77.2% of the contacts listed for follow-up, below the response target, while 46 health zones still had active transmission. Community deaths, resistance to post-mortem sampling and attacks affecting response routes add cases only after the easiest chance to interrupt them has passed. [E4]
The recorded death toll crossing 1,000 is a grim threshold, but the operational target lies elsewhere: reach the missing contacts before they become missing cases. Without safer access around response routes, the dashboard will remain both an alarm and an underestimate. [E1][E2][E4]
